Complaint Form for any Issues with Animals in Town
The following is required for us to be able to follow up your complaint. Please be as accurate as possible and ensure you provide a valid contact number for us to be able to call you to obtain further information. Alternatively, you can provide your email address and we can contact you that way.
Wherever possible, please include names of any other persons involved, the animals breed/colour/gender and the location of the incident. If you need to provide more information than the form allows, please make a note in the "Complaint Details" Section and an officer will be in contact with you.
- That on submission, I have lodged a formal complaint- That the above information is accurate to the best of my knowledge- That I may be contacted to confirm any details that may be required, and undertake to give full information to the Council as to this matter- That if this matter proceeds to court, I consent to being called as a witness to the truth of this complaint OR; whereby I do not consent to being called as a witness; I understand that this may result in the matter being unable to proceed any further due to insufficient evidence.
